Instructions to setting up a telemedicine appointment:

Please call our office at 303-862-3303 to schedule a telemedicine appointment.
- Please have insurance card ready and method of payment for copay and outstanding balances if applicable.
- Please also provide your preferred email address.
Please complete and electronically sign the following forms as applicable.

- Electronic HIPAA Compliant New Patient Packet
Our staff will call you prior to the appointment to ask questions concerning your history, symptoms and to review your medication list.
Wait for a text to initiate the telemedicine appointment with your physician
- Please note that the quality of the telemedicine visit seems best when you are connected to wifi and have sufficient bandwith.
- Please also be aware that the physician has scheduled office visit appointments and there maybe a small delay to the start of your appointment.

Allergy Shots

Allergy immunotherapy, also called “allergy shots,” is prescribed to modify the immune system’s response to allergens with the goal of minimizing and/or preventing an allergen initiated inflammatory response.
Allergy Drops & Tablets
